Photography Workshop Waiver & Release of Liability

Medical Release & Assumption of Risk

I, the undersigned (“Participant”), and, if applicable, the parent or legal guardian of the Participant (“Guardian”), acknowledge that participation in photography workshops and related activities conducted by Andrea Passarella Photography (“Program”) involves inherent risks.

These activities may include, but are not limited to, walking, standing for extended periods, outdoor shooting, travel between locations, interaction with photography equipment, and exposure to weather or environmental conditions. I understand that participation may result in physical illness or injury, including minimal, serious, catastrophic injury, or death.

I knowingly and voluntarily assume all such risks on my own behalf and, if applicable, on behalf of the Participant.

In the event of illness or injury, I authorize Andrea Passarella Photography to obtain necessary medical treatment for the Participant. I release and hold harmless Andrea Passarella Photography and its representatives in the exercise of this authority. I further understand and agree that I am financially responsible for any and all medical or related expenses incurred during participation in the Program, including travel to and from Program locations, whether or not the Program occurs.

Allergy Notice

I understand that the Program is not a controlled allergen environment. Participants may bring food or personal items that contain allergens such as nuts, wheat, dairy, eggs, or other substances. Andrea Passarella Photography is not responsible for adverse allergic reactions.

Please disclose any medical condition, allergy, or dietary concern below.

Insurance & Medical Information

I affirm that all medications the Participant is currently taking and any medications to which the Participant is allergic are listed below. I understand that Andrea Passarella Photography does not provide medication. The Participant is responsible for bringing and administering their own prescribed medications as directed.

Equipment Responsibility

I understand that Andrea Passarella Photography is not responsible for lost, stolen, or damaged personal property or photography equipment. Participants are solely responsible for securing their own equipment at all times.

Liability Release

For good and valuable consideration, the receipt and sufficiency of which are acknowledged, I grant permission for the Participant to participate in photography workshops and related activities conducted by Andrea Passarella Photography.

I, on my own behalf and, if applicable, on behalf of the Participant, hereby release, waive, and hold harmless Andrea Passarella Photography and its employees, officers, agents, representatives, contractors, and any venue where the Program takes place from any and all claims, demands, actions, damages, losses, liabilities, costs, or expenses (including attorney’s fees) arising out of or related to participation in the Program, whether caused by negligence or otherwise.

This release applies to all activities associated with the Program, including travel to and from Program locations. I acknowledge that this release does not constitute a guarantee that the Program will occur.
